First, we need to get the relevant pthread extensions on the system.
This typically means:

/opt/dcerpc/include/pthread_ext.h
/opt/dcerpc/lib/libpthread_ext.so.0

On Linux, to build this, you need to patch the glibc sources with the
contents of the dceext threads stuff...

Then, to build from scratch:

autoreconf -fis
./buildconf
./configure --prefix=/opt/dcerpc --with-pthread-ext-dir=/opt/dcerpc
make

To run the rpcd (which is required to end points for servers), these directories must be precreated:
$prefix/var/run
$prefix/var/rpc
